  • Please indicate which class(es) you would like to enroll in. Please list the complete course title and semester, if applicable (ie: Biology: Part 1). If you are requesting multiple classes, please use a semicolon to separate each course (i.e.: Biology: Part 1; English 12: Part 2).
  • If there is anything about you, your academic history, or your goals that we should know about, please let us know with as much detail as possible. If you prefer to schedule a consultation, please give us a call (435.649.3933).
    If you would like to set up a schedule to work with a teacher at EA, please fill out our Enrollment Form or give us a call. If you'd prefer to work on your course independently, you can always drop-in to work with teachers as needed.

  • Policies

  • Please read the following policies and select the checkbox at the bottom.

    • Course fees are $200/semester. If a student has a regular schedule at Educational Advantage, we will apply a one-time $50 credit to your account after the first month of tutoring.
    • The parent/guardian on file will receive an invoice for the course(s) within 15 days of enrollment. If the card on file is declined, student enrollment may be temporarily suspended until the course fee is paid in full.
    • It is the student's responsibility to request the correct class, and we strongly encourage approving any online course with their school counselor before submitting this form. If you need help selecting the correct course to satisfy your graduation requirements, you can call Educational Advantage and set up a consultation.
    • Students have 30 days to withdraw from their class to receive a 50% credit towards a different class. We do not issue refunds.
    • Upon course completion, students are required to complete a transcript request form, at which point a member of our EA staff will prepare and send an official transcript to their school.
    • Students have roughly nine months to complete their course. Upon enrollment, courses automatically assign 'due dates' for each assignment so that students are paced to complete their course before it expires. Students can adjust this 'due date' if they want to complete the course sooner. If students fall behind this pacing, their teacher will email them and the parent/guardian on file.
    • Students can request a course extension if needed. This is done by completing the online form found on our website and by paying the $50 extension fee. Students will receive an additional 90 days to complete their course. Course extensions cannot be purchased if a final exam has been requested.
    • All final exams must be proctored at Educational Advantage. Students can call, email, or stop by to schedule their final exam.
    • Students can retake a lesson quiz, most projects/assignments, and exams one time by completing resubmission form online our website. Quiz/assignment resubmissions have a $10 fee and exams have a $20 fee.
    • We strongly encourage students to have a mentor, tutor, or teacher to support their learning and progress, however it is not required.
    • As the parent/guardian submitting this registration form, I understand that it is my responsibility to provide the contact information for my ex-spouse so that both parents can be included in all correspondence regarding the student. If extenuating circumstances exist, I acknowledge that it is my responsibility to contact EA to disclose the details of all parent/guardian involvement.
    • In order to authorize your credit/debit card, a $.01 test will be processed.
    • In order for us to best accommodate your student, we may need to communicate with his/her school teachers or counselors. By agreeing to this policy, you authorize and permit EA staff to speak to your child’s school/teachers/counselors.

  • Payment Information

  • You can expect to receive an invoice for courses within 15 days of submitting this form. Students will be enrolled in their classes within 48 hours (business days) and will receive a welcome email with directions on how to log it to their course.
